Cardiac Rehabilitation

St. John's Home Health Services has developed a cardiac rehabilitation program to help patients and families with heart disease recover and return to optimal levels of functioning.

A nurse with training and experience in cardiac rehabilitation works with the physician, patient and family to develop an exercise program suited to the individual patient.

Home cardiac rehab services include:

  • Assessment of all systems with specific focus on cardiac and respiratory care.
  • Personal exercise training using safe exercise protocols, including access to cardiac monitoring.
  • Education about medication, risk factors for heart disease, lifestyle modifications, nutrition and stress management.
  • Communication with outpatient cardiac rehab programs as needed for a complete continuum of care.
  • Registered nurse on call 24 hours a day.

Other home care services available to patients include:

  • Infusion therapy
  • Pain management
  • Venipuncture
