Home Health Core Services
- Medical Supervision is provided by the patient's attending physician in consultation with the Home Health Services team.
- Skilled Nursing care is provided according to a physician's order. Care is available for patients needing home intravenous therapy, tube feedings, post-surgical care, wound management, diabetes management, cardiac care, respiratory care and many others. In addition, health education is provided for patients and their families.
- Home Health Aides, certified by the State of Illinois, provide personal care (such as bathing and grooming), assist with therapy exercises and assist home health nurses in providing patient care.
- Physical Therapy assists the patient with prescribed exercises and activities. The therapist can teach the patient, family or healthcare aide to help with appropriate treatment.
- Speech Therapy provides a program of treatment that may include feeding, communication and swallowing, as well as follow-up on the patient's progress.
- Occupational Therapy helps the patient in overcoming or living with disabilities. The therapist can teach the patient and family ways to adapt activities of daily living such as bathing, cooking and dressing.
- Social Work Services provides counseling to the patient and referrals to community agencies and resources.
