Home Health Service

St. John's Hospital Home Health Services provides care in the home as prescribed by a physician. Home Health Services can help promote, maintain or restore health, or provide pain and symptom control. We provide services to acutely, chronically ill patients and to patients recently discharged from the hospital who need nursing care and/or restorative therapy. Offices are located in the Springfield and the Decatur area. There is no restriction by diagnosis or age group. The frequency of home health visits is determined by the treatment plan established by the patient's physician and home health nurse. Certification

St. John's Home Health Services is licensed by the Illinois Department of Public Health as a home health agency. Home Health Services is also certified for participation in Medicare and Medicaid programs and accredited by the Joint Commission on Accreditation of Healthcare Organizations.
