Referrals

Referrals to St. John's Home Health Services are accepted from physicians, hospitals, nursing homes, community agencies and members of the community. Clarification of the services needed by the patient is always obtained from the attending physician before beginning care. We see patients in all of Sangamon and Macon counties, as well as parts of Christian, DeWitt, Logan, Macoupin, Menard, Montgomery, Morgan, Moultrie, Piatt and Shelby counties.
