Catalyst Scripts
St. John's Hospital is concerned about skyrocketing prescription costs. When given the opportunity to participate in a national program that could benefit our members, St. John's wholeheartedly accepted. As a result, this year we are pleased to provide members with a new, no cost opportunity to save money when purchasing prescription medications. The Catalyst Scripts card can be obtained by contacting Third Age Living at 544-6464, ext. 47756. A short application will be sent to you to fill in. There is NO cost for the Catalyst Scripts card.
Catalyst Preferred Vision Care
Catalyst Preferred Vision Care is a benefit program for members to help with the ever-increasing cost of eyeglasses and contact lenses. Through the program, members purchase prescription eye wear at a significant discount through a large network of participating optical providers. Members simply present the identification card at the time of service in order to receive the discounts through the plan. One identification card covers the entire family. There is a fee for the Catalyst Preferred Vision Care program. Please call Third Age Living at 544-6464, ext. 47756 to obtain an application.

Catalyst Dental Discount Program is a benefit program for members to help with the cost of dental care. Members simply present the identification card to a member dentist at the time of service in order to receive the discounts through the plan. The Catalyst Discount card can be obtained by calling Third Age Living at 544-6464, ext. 47756 for an application. A list of participating dentists will be mailed with the card. There is a fee for the Catalyst Dental Program.
