Final Affordable Care Act Enrollment Events at HSHS St. John’s Hospital This Weekend


Certified application counselors will be available at HSHS St. John’s Hospital to help individuals and families sign up for health care coverage through the Affordable Care Act (ACA). Individuals have until Jan. 31, 2017 to enroll.

Enrollment opportunities will be open to the public on the following dates:
 
Saturday, Jan. 28, 2017
10 a.m. – 10 p.m.
Yellow Rose Tea Room, HSHS St. John’s Children’s Hospital lobby, 415 N. Ninth St.
 
Sunday, Jan. 29, 2017
10 a.m. – 1 p.m.
Yellow Rose Tea Room, HSHS St. John’s Children’s Hospital lobby, 415 N. Ninth St.
 
Those planning on attending should be prepared to provide full names, birth dates, and Social Security numbers for all persons enrolling; proof of Illinois residency (i.e. Illinois driver’s license); and employer/income information (i.e. paycheck stubs, W-2).
