Plan ahead with advance directives


You have the right to make decisions about the health care you get now and in the future. An advance directive is a written statement you prepare that expresses how you want medical decisions made in the future should you not be able to make them for yourself. You do not need a lawyer to complete advance directives.

 
Advance directives and DNR orders include:
  • Health Care Power of Attorney (Naming a trusted relative or friend to communicate for you)
  • POLST/DNR Order (This order reflects the wishes of someone who is seriously ill or with a life-limiting illness. It states that cardiopulmonary resuscitation (CPR) will not be attempted if the heart or breathing stops.)
  • Health Care Surrogate (If you do not have a Health Care Power of Attorney, a "surrogate" or decision maker will be chosen for you under Illinois Law. )
