Access your health record online 

HSHS St. John’s Hospital offers our patients one of the easiest tools for health care management. MyChart is a free online personal health record that you can access securely from your home computer, laptop or mobile device. MyChart gives you the ability to view your medical record online and become an active participant in your own health and wellness. 

MyChart allows you to:

  • Retrieve updated health information electronically anywhere, anytime via personal computer, laptop, mobile device or smart phone.
  • Obtain a summary of your care during your hospital stay.
  • View your lab results.
  • View allergies, inpatient medications and vaccinations.
  • Schedule screening mammograms online. Instructions.

Getting Started – How it Works

There are two ways to register:
  1. Go to 
  2. Call toll-free at 1-800-236-6366
  3. Sign up at St. John's during your hospital registration*
  4. Visit St. John's Health Information Management Department (Medical Records)**

You will receive an activation code after signing up. To complete the registration process, you will be required to enter your activation code and follow the instructions on the screen.

* St. John's patients can sign up for MyChart during their hospital registration.
** Patients can also visit the Health Information Management (Medical Records) Department on the corner of Ninth and Carpenter streets anytime from 8 a.m. – to 4:15 p.m., Monday through Friday to sign up. All participants must have an email address and provide one form of identification.

To sign up a child, please visit the Health Information Department. A copy of his/her birth certificate or other legal proof of guardianship is required in addition to the guardian’s photo identification.
