    View Your Health Records From Your Computer or Mobile Device

    HSHS St. John’s Hospital now offers patients one of the newest and easiest tools for health care management. ChartMyHealth is a free, online personal health record that you can securely access from your home computer, laptop, mobile device or smartphone.

    ChartMy Health gives patients the ability to see a portion of their medical record, such as laboratory results*, current medication list and medical allergy list. 

    * Inpatient and Emergency Department patients only at this time.

    How do I sign up?

    St. John's patients can sign up for ChartMyHealth during their hospital registration. Patients can also visit the Health Information Management (Medical Records) Department on the corner of Ninth and Carpenter streets anytime from 8 am – to 4:15 pm, Monday through Friday to sign up. All participants must have an email address and provide one form of identification. 

    To sign up a child, please visit the Health Information Department. A copy of his/her birth certificate or other legal proof of guardianship is required in addition to the guardian’s photo identification.

    After signing up you will receive an email invitation from FollowMyHealth with a link to activate your account and create an authentication ID.

    NOTE: ChartMyHealth is best viewed in Internet Explorer 8 and higher and modern versions of Firefox, Google Chrome and Safari. A mobile app is available for both Android and Apple phones by searching for the “FollowMyHealth” app.
