Home Health Services
HSHS St. John's Hospital Home Health Services provides skilled services to 14 counties. If you have recently been hospitalized or have a medical condition that requires evaluation, continued monitoring or instruction/teaching about your condition, medications, IV infusion, tube feeding, wound care or home hospice our RN staff can provide this on an intermittent basis. We can also provide home psychiatric visits.
Home Health also provides home health aide assistance with personal care, all therapy services, a social worker for resource planning and chaplain services.
Patients must be home bound to receive Home Health services. (It must take a considerable and taxing effort to leave the home.) However, there are exceptions.
Home Health services are covered under Medicare, Medicaid, replacement policies and most private insurers.
Referrals for Home Health services are accepted from patients, physicians, hospitals, nursing homes, community agencies and members of the community. A physician's order and face-to-face encounter is needed for potential Medicare and Medicaid patients.
